WYSA Facility Use Agreement
Form requirement for any groups to rent fields, pavillions, or meeting room. Form must be completed and submitted to WYSA Board of Directors for approval. Outside groups must also provide proof of $1,000,000 liability insurance with Woodford Youth

WYSA Scholarship Request Form
All scholarship request will be reviewed by a committee. Some or all of the request may be granted. ALL paperwork requested should accompany the the request form.

WYSA Background Check Form
This form must be filled out by anyone wishing to coach, asst. coach, team parent, or help with any child at WYSA.

KYSA Risk Management
This form must be filled out by anyone wishing to coach, asst. coach, team parent, or help with any child at WYSA.

KYSA Coach Registration Form
This form must be filled out by anyone wishing to coach, asst. coach, team parent, or help with any child at WYSA.

Coaches Code of Ethics
All coaches, asst. coaches, and team managers must read and fill out the Coaches Code of Ethics.

Parents Code of Ethics
This is a copy of the Parents Code of Ethics that you agreed to when you filled out the applicaiton for your child to play.
